Stroke Warning Signs: Why CT Is Often Done First & When MRI May Follow

In the world of emergency medicine, we have a saying: "Time is Brain."

Every minute a stroke goes untreated, nearly 1.9 million brain cells die. That is why recognizing the warning signs and acting fast isn't just important—it is the difference between specific recovery and permanent disability.

At Magnus Diagnostics, we often see families rushing in with a loved one who "suddenly" couldn't speak or move an arm. In these moments, panic sets in.

This guide is to help you stay calm, recognize the signs, and understand exactly what happens inside the scanning room.


The Panic Button: Spotting a Stroke (B.E. F.A.S.T.)

You might have heard of FAST. In medical circles, we now use B.E. F.A.S.T. to catch even more symptoms.

  • B - Balance: Does the person have a sudden loss of balance or coordination?
  • E - Eyes: Have they lost vision in one or both eyes? (Blurry or blacked out).
  • F - Face: Ask them to smile. Does one side of the face droop?
  • A - Arms: Ask them to raise both arms. Does one arm drift downward?
  • S - Speech: Is their speech slurred or strange? Can they repeat a simple sentence?
  • T - Time: If you see ANY of these, go to the hospital immediately. Do not wait for it to "pass."

Why Do We Rush for a CT Scan First?

If you suspect a stroke, the doctor will almost always order a CT Brain immediately. Patients often ask me, "Why not an MRI? Isn't MRI better?"

Here is why we choose CT first:

  1. Speed: A CT scan takes 2 minutes. An MRI can take 20–30 minutes. In a stroke, those 20 minutes are critical.
  2. The "Bleed" Check: There are two types of strokes:
    • Ischemic: A clot blocks blood flow (Most common).
    • Hemorrhagic: A blood vessel bursts (Bleeding).
    • Crucial Difference: Treatment for a clot (blood thinners) would kill a patient with a bleed. A CT scan instantly shows us if there is fresh blood.

Bottom line: We need to know "Is there blood?" instantly so the Neurologist can start the right life-saving medication.


When Does the MRI Come In?

Once the emergency phase is over, or if the CT scan was normal but symptoms persist (a "mini-stroke" or TIA), we move to the MRI Brain.

The MRI is our "Deep Dive" tool. It tells us:

  • Exact Damage: It shows precisely which part of the brain died (infarction).
  • The "Penumbra": This is the area of "stunned" brain tissue around the stroke that might still be saved
  • Cause: It helps us find blocked arteries or small vessel disease to prevent a second stroke.

Patient FAQ

"My father had stroke symptoms, but the CT was normal. Why?"

This happens often in the first few hours of an Ischemic Stroke (Clot). The brain tissue hasn't died yet, so it looks normal on a CT. That is why doctors treat based on symptoms or order a follow-up MRI (Diffusion-Weighted Imaging), which picks up damage within minutes.

"Is the scan painful?"

Not at all. Both CT and MRI are painless. The hardest part is lying still when you are scared or confused. Our technicians are trained to be gentle and quick.


A Note to Families

If you have elderly parents at home, especially with high blood pressure or diabetes:

  1. Memorize the B.E. F.A.S.T. signs.
  2. Know your nearest hospital with a CT scanner.
  3. Don't try home remedies. Driving straight to the ER is the only right move.
